A home consultation puts medical judgement alongside the context of a person’s daily life. You will assess the reason for the visit, consider the available history and explain an appropriate management plan in language the patient can understand.
Working with nursing, diagnostics and patient-care colleagues, you help investigations and follow-up remain connected. Safe practice includes recognising the limits of a home setting, identifying when escalation is needed and documenting the reasoning behind the next step.

Discuss findings and suitable options with the patient. Arrange appropriate investigations, referrals or treatment and make the agreed plan and safety-netting clear.
Connect the follow-up
Document your findings and communicate relevant actions to the team. Recognise situations that need emergency or specialist care and use the appropriate escalation pathway.
